Giveaway Official Rules and Regulations (“Official Rules”) NO PURCHASE NECESSARY: Participants are not required to make a purchase or payment to enter the giveaway. ELIGIBILITY REQUIREMENTS: In order to be eligible, participants must provide their Name, Phone Number and Email and Consent to Communications from Mad Dog Customs, LLC dba Mad Dog Premium Golf Carts (“MD”). Entry can only be completed by filling out and submitting the above form, there are no other methods of entry. Entry is limited to legal residents of the United States of America, who are who are eighteen (18) years of age or older at the time of entry. MD has the right to verify the eligibility of any entrant. RULES AND REGULATIONS: Grand Prize Value: $4,500.00 The prize will be a golf cart, with a retail value of $4,500. The photos above are used for promotional purposes and may or may not be the exact golf cart which will be awarded to the winner of the giveaway. MD reserves the right to determine the specific golf cart offered in the giveaway. MD may, at its discretion announce the specific golf cart that will be awarded to the prize winner in or MD may offer a mystery golf cart, which has a retail value equivalent to the grand prize amount set forth in these Official Rules. The winner will be chosen at random by a randomized drawing, which will occur on or around January 1, 2026. MD reserves the right to shorten, extend, or otherwise modify the giveaway periods. Your entry is limited to this giveaway only and you will not be automatically entered into any other giveaway, raffle, or drawing that MD may in the future elect to conduct. One entry per person. Additional restrictions or conditions may apply. The winner will be selected by MD random. Your odds of winning are determined based upon the number of entries. Depending upon the location of the winner, transportation of the golf cart may be available. Each winner will be announced within 10 business days of the giveaway period ending for any particular giveaway. Announcement and instructions for prize collection will be sent via email message to the winner. Each entrant is responsible for monitoring his/her email account(s) for prize notification and receipt or other communications related to this giveaway. If a potential prize winner cannot be reached by MD within fifteen (15) days, using the contact information provided by the potential prize winner at the time of entry, that potential prize winner shall forfeit the prize. Upon the request of MD, the potential winner may be required to return an Affidavit of Eligibility, Release and Prize Acceptance Form and IRS W-9 form. If a potential winner fails to comply with these Official Rules, the potential winner will be disqualified and MD may, at its discretion select a new winner. Prizes may not be awarded if an insufficient number of eligible entries are received. Incidental expenses and all other costs and expenses which are not specifically listed as part of a prize in these Official Rules and which may be associated with the award, acceptance, receipt and/or use of all or any portion of the awarded prize are solely the responsibility of the respective prize winner. ALL prizes must be dispersed to winner(s) at MD’s showroom, located at 104 Ashourian Ave., Ste. 104, Saint Augustine, FL 32092. ALL FEDERAL, STATE AND LOCAL TAXES ASSOCIATED WITH THE RECEIPT OR USE OF ANY PRIZE IS SOLELY THE RESPONSIBILITY OF THE WINNER. ADDITIONAL LIMITATIONS. Prize is non-transferable. No substitution or equivalent prizes are permitted. MD and its respective affiliates and subsidiary companies, officers, directors, employees, members, managers, agents, and representatives are not responsible for any typographical or other errors in the offer or administration of the giveaway, including, but not limited to, errors in any printing or posting or these Official Rules, the selection and announcement of any winner, or the distribution of any prize. Any attempt to damage the content or operation of this Giveaway is unlawful and subject to possible legal action by MD. MD reserves the right to terminate, suspend or amend the Giveaway, without notice, and for any reason, including, without limitation, if MD determines that the Giveaway cannot be conducted as planned or should a virus, bug, tampering or unauthorized intervention, technical failure or other cause beyond MD’s control corrupt the administration, security, fairness, integrity or proper play of the Giveaway. In the event any tampering or unauthorized intervention may have occurred, MD reserves the right to void suspect entries at issue. MD and its respective affiliates and subsidiary companies, agents, and representatives, and any telephone network or service providers, are not responsible for incorrect or inaccurate transcription of entry information, or for any human error, technical malfunction, lost or delayed data transmission, omission, interruption, deletion, line failure or malfunction of any telephone network, computer equipment or software, the inability to access any website or online service or any other error, human or otherwise. INDEMNIFICATION AND LIMITATION OF LIABILITY. MD AND ITS AFFILIATES AND SUBSIDIARY COMPANIES DO NOT MAKE ANY WARRANTIES, EXPRESS OR IMPLIED, AS TO THE CONDITION, FITNESS OR MERCHANTABILITY OF THE PRIZE.
